New Patients
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We are a bilingual office and are honored to care for you.
What to bring to your
first appointment
- Insurance card(s)
- List of all current medications and allergies
- X-rays, MRIs, or other diagnostic studies, including nerve conduction tests which have been performed elsewhere
- Any relevant medical records or test results
- Any paperwork or questionnaire mailed to you in advance
- Copy of acceptance letter for any worker’s compensation claim

Insurance
Klamath Orthopedic Clinic will partner with you in securing reimbursement from your insurance carrier(s) for your orthopedic care.
Most insurance plans have specific guidelines. If necessary, contact your primary care physician for authorization or referral. We will follow your insurance company’s guidelines and collect any co-pays at the time of service.
Please be familiar with your responsibilities and your insurance company’s requirements.
If your treatment is due to a motor vehicle accident or worker’s compensation injury, please provide us with the name and address of the responsible insurance company and your claim number.
It is essential that we have the referrals if required by your personal insurance carrier in the event your worker’s compensation claim is denied or your motor vehicle benefits are exhausted.

Prescription Refills
Prescription refills, including narcotics, are handled by the office only during regular business hours when our full records are available.
Renewals requested at other times will be filled only for extreme or emergency circumstances. Therefore, please plan ahead for medication needs you may have at night or over the weekend. Please allow 24-48 business hours for refills of maintenance medications.
